Dear Patient,

We are writing to inform you of an urgent and significant change that will directly impact your access to care with Ascend Vision and its affiliated providers.

After more than 20 years as a participating provider and partner, Ascend Vision will no longer be in network with certain health plans administered by Premier Eye Care. This change is not by our choosing. Recent reimbursement reductions and administrative decisions by Premier Eye Care have made it unsustainable for us to continue participating while maintaining the level of care our patients expect and deserve. This decision will result in the loss of access to some of the most experienced ophthalmic surgeons, pediatric ophthalmologists, neuro ophthalmologists, and optometrists serving patients in this region.

The affected plans include Wellcare, CarePlus, Humana, Ultimate, and Devoted.

As a result, our providers will be considered out of network as follows:

We understand this is disruptive and frustrating. Appointments and surgeries scheduled around these dates will need to be canceled unless alternative payment arrangements are made in advance. Out of network means your insurance may not cover services with our physicians, or you may be responsible for a significantly higher portion of the cost.If you value continuing your care with your current provider, we strongly encourage you to contact us immediately. Our team can help you explore self-pay options and discuss alternative health plans we accept that may allow you to remain under our care. We are committed to helping you maintain continuity and avoid interruption in your treatment.

You may also contact your health plan to understand your options. If remaining with Ascend Vision is important to you, we encourage you to let your plan know that you want to continue seeing your current provider. Patient feedback does matter, and plans do take it into consideration.

If you decide not to continue your care with us, your health plan can assist you in identifying another in-network provider.
